All potential students must fill out and submit the application. A Student Application must be submitted with the student’s IEP and any previous ABA Assessment to assure enrollment the following fees must be paid prior to enrollment.

Florida Certification of Immunization or Immunization Exemption form

The Form DH 680, Florida Certification of Immunization, must be used to document receipt of immunizations required for entry and attendance in Florida schools, childcare facilities, and family daycare homes. For more information, please follow this link

School Entry Health Exam
Form (DH 3040)

Students in grades prekindergarten through twelfth grade who are making their initial entry into a Florida school must present a record of a school-entry health examination completed within one year before enrollment. For more information, please follow this link.
